This position will be mainly for the hospice program and will be a back up for some home health visits.
Hospice Follow Up
We are hiring Case Manager team members who can play the role of back up admission nurse, evaluations, follow up visits.

Home Health Case Manager
We are hiring Case Manager team members who can play the role of case manager on a per diem basis. We are looking for members who can help with timely Supervisory Visits, ROC & Recertification. These visits are vital part in coordinating the game plan for the patient with the follow up nurse. Teamwork with the patient's clinical team is important for this role.
RN's MUST have knowledge:
- Must have experience in hospice and home health
- Hospice Admissions, case management, reporting
- OASIS (Admissions, Resumptions, Recerts, D/Cs)
Special Skills with any of the following is also a plus:
- Port-a-Cath, PICC, Peripheral, or Central Lines
- IV or TPN Administration
- Chemo Therapy Certified
